employment relationships with each organization and will receive
individual payroll compensation from each entity based on the time
worked and services provided for their respective programs. This
means you will be an official employee of multiple affiliated
companies, with hours, responsibilities, and compliance obligations
tracked separately for each. This role is structured as a 16-hour
per week, part-time position. While day-to-day supervision and
scheduling will be coordinated centrally to support integrated
care, each employer will manage its own onboarding, HR, and
compliance processes in accordance with applicable laws and

Healthcare. Together, these organizations serve economically
disadvantaged, disabled, and high-risk individuals through
integrated behavioral health, supportive housing, and primary care
programs. DIVISION: Operations / Clinical Services ACCOUNTABLE TO:

SCHEDULE Tuesday and Thursday 8:00 AM 5:00 PM (16 hours per week)
On-call availability may be required for after-hours emergencies.
COMPENSATION Hourly pay ranges from $90.00 to $150.00, commensurate
with experience, expertise, verified credentials, and available

between program sites in Maryland. No additional compensation is
provided for travel. PHYSICAL DEMANDS Regularly walk, stand, or
stoop. Occasionally lift, carry, push, pull, or move objects up to
25 pounds. Drive a motor vehicle as part of duties. WORKING
CONDITIONS Community-based and outpatient clinical environments.
Regular interaction with residents, clients, staff, external
providers, and emergency personnel. Exposure to communicable
diseases due to patient care responsibilities. SUMMARY OF POSITION
RESPONSIBILITIES The Physician provides comprehensive primary and
integrated healthcare services to individuals enrolled in
outpatient behavioral health, psychiatric rehabilitation, substance
use treatment, and primary care programs. This role is responsible
for performing medical evaluations, managing acute and chronic
health conditions, prescribing medications (including MAT when
indicated), and coordinating care with interdisciplinary teams. The
Physician ensures that all services meetCOMAR regulations, OHCQ
standards, and Maryland HealthGeneral Article Title 19 requirements
while supporting integration of behavioral and somatic health
services. The Physician is authorized to bill CPT codes9920299205
and 9921199215 for eligible services rendered. This position plays
a critical role in bridging the gap between primary care, mental
health, and substance use treatment, ensuring that clients receive
safe, person-centered, and regulatory-compliant services. SCHEDULED
DUTIES AND RESPONSIBILITIES Conduct face-to-face medical
evaluations, wellness visits, and follow-up care for enrolled
clients. Diagnose and manage acute and chronic health conditions;
prescribe medications within scope of practice. Perform initial and
ongoing MAT evaluations, including induction, stabilization, and
monitoring, as needed. Order, review, and interpret diagnostic
tests, labs, and urine drug screens. Review, document, and update
comprehensive medical histories, including conditions, allergies,
communicable diseases, and medications. Develop individualized
treatment and follow-up plans, including referrals to primary care
providers or specialists. Educate clients and families on disease
prevention, wellness strategies, relapse prevention, and safe
medication use. Participate in interdisciplinary team meetings,
case reviews, and staff trainings. Document all encounters in the
EHR (ICANotes) in compliance with COMAR, Medicaid, and payer
standards. Support audit readiness, licensing inspections, and
quality assurance initiatives. UNSCHEDULED DUTIES AND
RESPONSIBILITIES Respond to after-hours emergencies when delegated
or on-call. Act as clinical consultant for complex cases requiring
coordination with outside providers. Participate in peer reviews,
documentation audits, and policy updates. Provide staff coaching
and consultation on medication protocols, infection control, and
resident safety. Represent the organization in family meetings,
licensing reviews, and community partner engagements. Carry out
additional duties as assigned by the Medical Director or Operations
Leadership. COMPETENCIES AND SKILLS Strong clinical skills in
primary care, integrated care, and addiction medicine. Knowledge of
COMAR, OHCQ, CARF, SAMHSA, and DEA regulations. Proficiency in EHR
systems (ICANotes preferred) and CPT coding. Effective
communication, teamwork, and conflict-resolution abilities. Crisis
response and clinical decision-making under pressure. Commitment to
trauma-informed, person-centered care. LEVEL OF EDUCATION /
TRAINING / QUALIFICATIONS Active, unrestricted Maryland Board of
Physicians license (MD or DO). Doctor of Medicine (MD) or Doctor of
Osteopathic Medicine (DO) degree from an LCME- or COCA-accredited
medical school. Successful completion of an ACGME- or
AOA-accredited residency program in: Family Medicine, Internal
Medicine, or Medicine-Pediatrics (preferred depending on population
served). Board certification or board eligibility in Family
Medicine or Internal Medicine (board certification preferred).
Valid DEA registration and Maryland CDS (Controlled Dangerous
Substances) registration. Minimum 23 years of post-residency
clinical experience in: Primary care, Community health, Integrated
care, behavioral health, or complex patient populations
(preferred). Experience coordinating care with external providers,
specialists, hospitals, and pharmacies. Experience with MAT
services (buprenorphine, naltrexone, etc.) strongly preferred.
Current BLS/CPR certification (ACLS preferred but optional
depending on scope). Ability to obtain and maintain professional
liability/malpractice insurance. Must be eligible for credentialing
with Medicaid, Medicare, and commercial payers.
