Registered Nurse (RN) | Case Manager Location: Clinton, MD Agency: Cynet Health Pay: $2,406 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/23/2026 About the Position Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: 2 years of acute hospital experience required. Travel experience required. License: MD license required. Certifications: BLS required. Must-Have: Discharge planning experience required. Ability to handle up to 20 patient assignments per day. Ability to float to medical units, ICU/CCU, and maternal child health units. Every other weekend required. Description: The position is for a Case Manager RN. Open to LMSW, MSW, or LCSW candidates. Candidates must have inpatient case management experience. Experience with discharge planning is essential. The role involves managing a high patient load. Weekends required, with two 2-day weekends in a 6-week rotation. The daily census is expected to be 180 with 220 beds in total. Interdisciplinary support includes physical therapy, respiratory services, and social services. Candidates should possess skills in care coordination and utilization of criteria for admissions and continued stay reviews. Strong charting skills are necessary, with experience in relevant charting systems preferred. The role includes handling diverse patient age groups, including infants, adults, and geriatrics.
