Therapist (PT) to provide high-quality rehabilitative care in a
clinical or skilled nursing setting. This role focuses on helping
patients improve mobility, manage pain, and regain independence
through individualized treatment plans. The ideal candidate is
patient-focused, detail-oriented, and committed to delivering
effective therapy outcomes. Key Responsibilities Evaluate patients
and develop individualized treatment plans Provide therapeutic
exercises and rehabilitation techniques Monitor patient progress
and adjust treatment plans as needed Educate patients and families
on recovery and mobility strategies Maintain accurate and timely
documentation Collaborate with interdisciplinary healthcare teams
Ensure compliance with safety and clinical standards Requirements
Active Physical Therapist (PT) license Previous experience in a
clinical or rehabilitation setting
